I further confirm that I have read and understood and agree to all the above and all the information given and I have personally informed all parties in my booking who agree with the information given and we will all comply with any other related requirements, measures and protocols notified to us by the Hotel. 

Guests and staff health and safety is always our Hotels number one priority. The Hotel has implemented in accordance with EU and local government guidelines on the management of Coronavirus appropriate procedures to safeguard its guests and staff health and safety whilst guests are staying at the Hotel. It is, therefore, essential that all guests using the Hotel facilities strictly follow measures and guidance given by the hotel to prevent infection and virus transmission to a maximum 
In this regard, while staying at the Hotel you must respect any and all related healthcare measures (indicatively including physical distancing, hand washing, use of face masks). 
You must stay in your room if you experience at any time during your stay at the Hotel minor symptoms such as cough, headache, mild fever, and get in touch with the Hotels reception immediately. Upon departure, you are explicitly requested to immediately notify the Hotel if you develop symptoms associated with COVID-19, or obtain a positive test result for COVID-19, within 14 days following departure. 

Ich bin damit einverstanden, dass das Hotel zum Schutz der öffentlichen Gesundheit aktualisierte Unterlagen aller Gäste, die sich im Hotel aufhalten, bewahren muss - Name, Nationalität, An- und Abreisedatum, Kontaktdaten (Telefon, E-Mail), damit einen Kontakt möglich ist, falls zu einem späteren Zeitpunkt ein Coronavirus-Fall festgestellt wird. Die Datenschutz-Grundverordnung wird von allen Mitarbeitern eingehalten und die Angaben der Gäste werden aus Gründen des Gesundheitsschutzes in Dateien gespeichert.

Η υγεία και η ασφάλεια των πελατών και του προσωπικού είναι πάντοτε πρωταρχική προτεραιότητα του Ξενοδοχείου μας. To κατάλυμα έχει εφαρμόσει σύμφωνα με τις κατευθυντήριες γραμμές της Ευρωπαϊκής Ένωσης και της Ελληνικής Κυβέρνησης για τη διαχείριση του Covid-19, τις κατάλληλες διαδικασίες για τη διαφύλαξη της υγείας και ασφάλειας των πελατών και του προσωπικού του, ενόσω οι πελάτες διαμένουν στο Ξενοδοχείο. Επομένως, είναι απαραίτητο όλοι οι πελάτες και επισκέπτες που χρησιμοποιούν τις εγκαταστάσεις του Ξενοδοχείου να ακολουθούν αυστηρά τα μέτρα και τις οδηγίες που παρέχονται από το Ξενοδοχείο, για την πρόληψη της μόλυνσης και της μετάδοσης ιών στο μέγιστο δυνατό βαθμό.
Συνεπώς, κατά την παραμονή σας στο Ξενοδοχείο πρέπει να τηρείτε όλα τα σχετικά υγειονομικά μέτρα (ενδεικτικά συμπεριλαμβανομένης της τήρησης των φυσικών αποστάσεων, της απολύμανσης των χεριών και της χρήσης μασκών προσώπου).
Πρέπει να παραμείνετε στο δωμάτιό σας εάν αντιμετωπίσετε οποιαδήποτε στιγμή κατά τη διάρκεια της διαμονής σας στο Ξενοδοχείο ήσσονος σημασίας συμπτώματα, όπως βήχα, πονοκέφαλο, ήπιο πυρετό και να επικοινωνήσετε αμέσως με την υποδοχή (reception) του Ξενοδοχείου. Μετά την αναχώρησή σας, παρακαλείσθε ρητώς να ενημερώσετε αμέσως το Ξενοδοχείο στην περίπτωση που εμφανίσετε συμπτώματα που σχετίζονται με τον COVID-19 ή εάν έχετε θετικό αποτέλεσμα σε εξέταση στην οποία υποβληθήκατε για τον COVID-19, εντός 14 ημερών από την αναχώρηση σας.

Zgadzam się, że w celu ochrony zdrowia publicznego hotel musi na prowadzić aktualny zapis wszystkich gości przebywających w hotelu – imię i nazwisko, narodowość, datę przyjazdu i wyjazdu, dane kontaktowe (telefon, adres e-mail), tak aby komunikacja była możliwa w przypadku koronawirusa w późniejszym terminie. RODO jest przestrzegane przez wszystkich pracowników, a informacje o gościach będą przechowywane w aktach ze względu na ochronę zdrowia publicznego.

I acknowledge that my personal data and information will need to be used by the Hotel, as prescribed under the abovementioned protocols (and particularly in cases of potential or confirmed infections) and expressly agree thereon. Ich erkläre und bestätige, dass ich ordnungsgemäß darüber informiert wurde, dass das KOALA HOTEL die gesetzlichen Bestimmungen bezüglich der speziellen Gesundheitsprotokolle erfüllt (gemäß dem Gemeinsamen Ministerialbeschluss 1881/29.5.2020 bezüglich der speziellen Gesundheitsprotokolle, die für die Tourismusunternehmen im Hinblick auf die Maßnahmen gegen Covid-19 gelten werden) und erkenne an, dass das Hotel nicht haftbar gemacht werden kann (für die Wiederherstellung von Schäden oder anderweitig), solange die in den besagten Gesundheitsprotokollen vorgesehenen Maßnahmen eingehalten werden.

Dichiaro e confermo di essere stato debitamente informato che KOALA HOTEL soddisfa le disposizioni di legge in materia di protocolli sanitari speciali (come previsto dalla Decisione Ministeriale Congiunta 1881 / 29.5.2020 relativa ai protocolli sanitari speciali che si applicheranno alle imprese turistiche alla luce delle disposizioni contro Covid-19) e riconosco che l'Hotel non potrà essere ritenuto responsabile (per il ripristino di danni o altro), purché siano rispettate le misure previste da detti protocolli sanitari.

Oświadczam i potwierdzam, że zostałem należycie poinformowany, że KOALA HOTEL spełnia przepisy ustawy dotyczące specjalnych protokołów opieki zdrowotnej (zgodnie ze wspólną decyzją ministerialną 1881/29.5.2020 w sprawie specjalnego protokołu opieki zdrowotnej które będą miały zastosowanie do przedsiębiorstw turystycznych w świetle środków przeciwko Covid-19) i uznają, że Hotel nie ponosi odpowiedzialności (za przywrócenie szkód lub w inny sposób), o ile środki przewidziane w tych protokołach opieki zdrowotnej są przestrzegane.

The undersigned hereby (a) declare and confirm that I have been properly informed that the Hotel satisfies the provisions of the law regarding the special healthcare protocols (as provided under the Joint Ministerial Decision 1881/29.5.2020 regarding the special healthcare protocols that will apply to the touristic enterprises in light of the measures against COVID-19) and (b) acknowledge that the Hotel shall not be held liable (for the restoration of damage or otherwise), as long as the measures provided under said healthcare protocols are complied with.  

Ich bestätige, dass ich und der/die Mitbewohner meines Zimmers keine Symptome der Covid-19-Krankheit haben, dass wir nicht in Quarantäne sind, dass wir alle notwendigen Vorsichtsmaßnahmen gegen die Epidemie ergreifen und dass wir auf eigenes Risiko reisen.

Ο υπογράφων διά της παρούσης (α) δηλώνω και βεβαιώνω ότι έχω ενημερωθεί προσηκόντως ότι το Ξενοδοχείο πληροί τις προϋποθέσεις του νόμου για τα ειδικά πρωτόκολλα υγειονομικού περιεχομένου (κατά τα οριζόμενα στην υπ΄αριθμόν 1881/29.5.2020 Κοινή Υπουργική Απόφαση «Ειδικά πρωτόκολλα υγειονομικού περιεχομένου βάσει των οποίων λειτουργούν οι τουριστικές επιχειρήσεις στο πλαίσιο της λήψης μέτρων έναντι του COVID-19») και (β) αναγνωρίζω ότι το κατάλυμα ουδεμία ευθύνη φέρει (προς αποκατάσταση ζημίας ή άλλη), εφόσον τηρούνται τα μέτρα των εν λόγω υγειονομικών πρωτοκόλλων.

Confermo che io e chi condivide la mia stanza non abbiamo sintomi della malattia Covid-19, non siamo in quarantena, prenderemo tutte le precauzioni necessarie contro l'epidemia e che viaggiamo a nostro rischio.

Accetto che per la protezione della salute pubblica, l'hotel debba tenere un registro aggiornato di tutti gli ospiti che soggiornano presso l'hotel - nome, nazionalità, data di arrivo e partenza, dettagli di contatto (telefono, e-mail), in modo che sia possibile la comunicazione se un caso di coronavirus viene identificato in un secondo momento.
